By filling out this form, I agree to the following: 

As my child participates in the Vacation Bible School activities for the week of June 22nd - 26th, 2026, at Foundation Church, I understand that photos and videos may be taken of the event.  I understand - and authorize for my child(ren) - that these photos and videos may be used on our website, social media channels, or printed publications.  I understand that these photos and videos will not have names of my children included or tags of any kind that would identify my children by name. 

Furthermore, if my child is a participant in the VBS Music on Sunday, June 28th, I authorize their image (photo and/or video) to be part of the online presence for the normal service.  Again, with the understanding that there will be no names or tags identifying my child in the service video.   

I understand that if I choose not to acknowledge this waiver, my child can still participate in Vacation Bible School, but not on the stage for the VBS Music on Sunday, June 28th.
